            F – Details of compensatory days of rest

            G – Details of leave in any calendar year

            H – Details of leave in current calendar year

            I  – Details of Overtime

            J  – Return of reportable injury

            K – Return of minor accidents

            L – Details of leave with wages of employees to be displayed in notice board

            M – First Notice for PME(Preliminary Medical Examination)

            N – 2^nd Notice for PME(Reminder) after 10 days of issuing Form-M

            O – Report of medical examination

            P – Details of various tests conducted during PME

            T – Annual Return of the mine to be sent to Chief Inspector

            U – Report of Inspections held by Workman Inspector

2.      The Payment of Gratuity Act, 1972

Form   A – Notice of opening of establishment to be submitted to the controlling authority

B – is sent to ALC/RLC regarding change of name / address / owner or manager of the mine.

C – Notice of closure of establishment

D – Notice for excluding husband from family

E – Notice of withdrawal of notice for excluding husband from family

F – Nomination form after one year of continuous service (Section-6)

G/H – Change of nomination   

I – Claim for gratuity by self

J – Claim for gratuity by self

K – Claim for gratuity by legal heir

L – Information to employee regarding payment of gratuity

M – Notice rejecting claim for payment of gratuity

N – Application for direction to controlling authority

O – Notice for appearance before the controlling authority

P – Summons to appear before the controlling authority

R – Notice for payment of gratuity

S – Notice for payment of gratuity as determined by Appellate authority

T – Application for recovery of gratuity

U – Abstract of the acts and rules
