N – Form for return of strike/lockout to be
submitted to ALC
E – Notice of change in service condition by
the employer.
Q – Notice/Report of closure of establishment
by the employer

Form – I – Application by the employer for
registration of establishment
II – Certificate of Registration issued by the
registering authority

The Coal Mines Pension Scheme, 1998:
PS-1 – Those who were not member of CMFPS-71 but
were member of CMPF will opt for pension through PS-1
PS-2 – Those who were member of both CMFPS-71
& CMPF but had retired between 01-04-94 to 31-03-98 will opt for becoming
member of CMPS-98 through PS-2.
PS-3 – Family declaration form.
PS-4 – Nomination for pension
PS-5 – Return of monthly contribution of
employees to CMPF office.
PS-6 –Claim form for pension/different benefits
