Statutory Forms widely used under different Acts/Rules/Schemes.


1.      The Industrial Disputes Act, 1947
Form – H – Form for in-house settlement between Management and the union(s)
             N – Form for return of strike/lockout to be submitted to ALC
             E – Notice of change in service condition by the employer.
             Q – Notice/Report of closure of establishment by the employer
2.      The Contract Labour Regulation & Abolition Act, 1970
Form – I – Application by the employer for registration of establishment
             II – Certificate of Registration issued by the registering authority
            VIA- Report of Commencement/Closure of work to be submitted by the contractor
            VIB - Report of Commencement/Closure of work to be submitted by the employer
             XII – Register of contractor maintained by the principle employer
            XVIII – Muster Roll of contractor workers maintained by the contractors
            XXV – Annual Return by the principle employer to be submitted to the inspecting
   Authority
3.      The Coal Mines Pension Scheme, 1998:
PS-1 – Those who were not member of CMFPS-71 but were member of CMPF will opt for pension through PS-1
PS-2 – Those who were member of both CMFPS-71 & CMPF but had retired between 01-04-94 to 31-03-98 will opt for becoming member of CMPS-98 through PS-2.
           PS-3 – Family declaration form.
           PS-4 – Nomination for pension
           PS-5 – Return of monthly contribution of employees to CMPF office.
           PS-6 –Claim form for pension/different benefits
4.      LTC/LLTC (Non-executives)
  Form A – Declaration containing details of family & dependency
            B – Advance form against LTC
            C – Checklist/verification of advance
            D – Final LTC bill submission
5.      Coal Mines Privident Scheme:
 Form H – Allotment of PF No/account
            A – Nomination/declaration form
            M – Change of nomination form
            Schedule-‘C’ – Details of family members
